Appointment Information
How do I make an appointment?

Making an appointment with SDRC is easy. Simply request an appointment with us, and we will walk you through the rest of the steps.
When you request an appointment, we receive an email in our secure email system. We will reach out to you within about 5 business days and send you several questionnaires to fill out about the person being referred. We will ask you to email the paperwork back to us, along with a series of pictures.
After we receive the documents and pictures, we will send you a link to our appointment calendar and you can choose the appointment that works best for your schedule.
Appointment Costs:

SDRC evaluations are $175 per half hour unit and can be paid by debit or credit card. This will include an interdisciplinary assessment; full report, including specific accommodations for your child; and access to training and resources for addressing symptoms of prenatal substance exposure with an emphasis on fetal alcohol spectrum disorders. We are currently private-pay only, but can work with you if finances will prevent you from receiving an evaluation.
